Faced with problems ranging from misfires to barrels breaking off, the state has asked gun maker Smith & Wesson to replace hundreds of sidearms carried by probation and corrections officers.
None of the revolvers have failed in the line of duty, and for now, the department is keeping the guns in service. But in testing, about one in four revolvers didn't fire when the trigger was pulled. In some cases, the barrel of some models broke off when the gun was fired.
"In one sense it's funny," said Chief Deputy Correction Secretary Dan Stieneke. "In another, it's alarming."

Check out Pachmayr Compacts for grips. I find them a bit better for recoil reduction and concealment than the factory hogues.

Should have done what 90% of the other agencies do and purchased Glocks.
Colt calls the same part an ejector rod. Don't recall offhand which way it's threaded, could be backwards.
Another possibility is a burr on one of the crane lock detent parts in the recoil shield {where the cylinder locks to the frame at the back}.

Smith changed the extractor rod threads in 1961 on many guns. from right to left to help prevent this from happening. If your extractor rod has a small milled stripe immediately behind the knurled section at the tip it is a lefty. The Model 38 (Bodyguard with the cool shouded hammer) is a J frame, and I'm not really savvy on their mod and production history.
I gave the glocks a try, but they just don't work for me. They're fine, reliable guns, but not where the budget goes.
Maybe they weren't designed to take the place of hammers and nightsticks.

Too much torque during barrel install maybe ?? Sabotage at the factory ? A dumbass that doesn't know how to use a floating beam wrench maybe ?:o)

I have a S&W "Old Army" Model No. 2 made in 1869 that still worked well until my source for black powder .32 rimfire cartridges dried up.
Buy Ruger, they have NEVER been a member of the "Gun Grabber" lobby like S and W have been!
