Judith Anne:

Your reply to 2ndreconmarine about the numbers being all over the place indicates his original projections were correct is actually staggering to contemplate.

However, I do have a couple of question for you, 2ndreconmarine, or anyone else that would be happy to reply: Although I understand the basic concept of the statistical analysis used by 2ndreconmarine, 1) Does or can the formulation take into account the cultural aspects of the demographics of the outbreak area (i.e. large households, methods for treating the ill, and burial practices)? 2)Does or can the formulation account for variable incubation periods (I've read of incubation periods ranging from 7 days to 21 days)?

I believe that we all believe the reported numbers are not correct. An extrapolation of basic thought from a lay person as follows: 1 person with Marburg has 5 other immediate family members. Each of those family members has a sphere of influence of 10 community members...etc.

As I recall the majority of the original 95 reported cases were children. How many people cared for the children when they were sick? How many other people were the children around with runny noses, sweat, coughing, sneezing, etc. before they were bed ridden?

Since it's apparent that this outbreak hasn't burned itself out, and seemingly inconsistent with other outbreaks - where are the numbers in reality?

Thanks in advance for any insight from anyone.

Pretty chilling, isn't it? The cold war is over and the bug/virus "war" has begun. I just wish we could get some consistent, believable numbers and information from the media (such as it is) and/or the powers that be.

At one point I read that each case was presumed to have 5 contacts, which is interesting in light of your post.

14 of the original 95 children were CONFIRMED to have Marburg by CDC testing, so projections are based on 14 original cases as of January, 2005.

I know 2ndreconmarine's projections were staggering. I don't know that cultural variables are included in his projections, primarily because Marburg began in Uige city, Uige province.

I DO think that most Angolans know to stay away from the obviously sick people, and I think they probably do, as much as possible--in some cases, people are not known to be sick with Marburg.

Another thing I keep in mind is that in Uige province, there are more than 3 million people. I don't think 500 are more than an unimportant drop in the bucket to Angolan authorities, and I believe they still think it will "burn itself out."

I also think most people with Marburg are not going to hospitals for any reason. Why die there, neglected among cold strangers? Why not go into the rural countryside, where a brave relative may go with you, and accompany your death? Just my own musings, here.

Storm is over, had FoxNews on in the background, and Donna Fiducia (I think) mentioned the presumed Ebola outbreak in the Congo.

Nothing was said about Marburg in Angola, of course.

This area has had Ebola outbreaks before, so that's the main reason they think it's Ebola, instead of wondering if it's Marburg.

The points in the article about Ebola that caught my attention are

Marburg in Angola is uncontained.

The Congo Republic thinks this is Ebola because of previous Ebola outbreaks in the same area.

The Congo Republic is certain that they will contain their outbreak, unlike Angola.

WHO, which has 20 people in Angola, still has no plans to withdraw because the Angolan Marburg outbreak is still not contained.

That is absolutely disgusting. Proof positive that the Angolan government is manipulating causes of death thus skewing the Marburg death count. Malaria.. yeah right.

MARBURG HEMORRHAGIC FEVER - ANGOLA (40)
***************************************
A ProMED-mail post

Angola- [Republic of the] Congo: Officials review fight against Marburg virus
---------------------------------------------------
[This report does not necessarily reflect the views of the United Nations]

Angolan and Congolese medical officials, as well as representatives of 2 UN agencies, have reviewed ways to stop the spread of the deadly Marburg virus across their borders. "As neighbouring countries, we need to continue working together, exchanging ideas and information in order to overcome this epidemic," the officials said on Friday at the ROC border town of Masabi.

The officials, who included representatives of the UN Children's Fund and the UN World Health Organization, discussed a report on the fight against the haemorrhagic fever, which started in the Angolan province of Uige, killing at least 200 people. It has spread to 8 other provinces in Angola.

Marburg is an Ebola-like virus transmitted by direct contact with body fluids. Symptoms of the disease include high fever, haemorrhaging, vomiting, and diarrhoea, which appear after an incubation period of 2-21 days. There is no known cure or vaccine.

The ROC doctors who participated in the Masabi meeting were from the capital, Brazzaville and the commercial port city of Pointe-Noire. They were led by the director-general of health, Dr Damase Bozongo, while the Angolan delegation was led by Dr Francisco Bungo, the chief representative of the health services in Angola's 2nd Region. "We made all necessary provisions in the town of Cabinda [the northern Angola enclave] to stop the spread of this epidemic to neighbouring towns," Bungo said.

He added that sensitisation campaigns were being done at in the army, schools, churches and through political parties. Safety measures, including quarantining and testing, had been put in place at the country's airport and ports in order to avoid infections in or out of the country, he said. "When we received the information of a case in Cabinda, we got together with medical authorities of our neighbouring countries in order to stop the spread," Bozongo said.

Commenting on the health safety measures so far, the WHO representative in ROC, Dr Adamou Alzouma Yada, said, "Collaboration between these 2 countries in fighting the virus was a very good initiative." He added that the spread of the disease would be "very dangerous and fast" because of cross-border movement of people. He advised the 2 governments to improve their communication in order to monitor the virus better.

ROC has deployed teams in Pointe-Noire and in the towns of Banga-Ntaka, Kimpessi, Kimongo and Dolisie in the south western part of the country to monitor the situation.

It has spread to 8 other provinces in Angola.

Spread? This seems like new specific information. Are they single "isolated" cases which have terminated out in each of the other 8 provinces, or what? We had heard there were other cases in the country, but this is the first I have heard "8 other provinces" mentioned!

"When we received the information of a case in Cabinda, we got together with medical authorities of our neighbouring countries in order to stop the spread," Bozongo said

Cabinda is roughly 400 miles as the crow flies northwest of the city of Uige - and probably a number of days travel. One also has to cross Zaire (or go via air or boat around it) to get to Cabinda. What is the likelihood of further spread, especially if a somewhat circuitous route for an infected person was taken and the infection originated in Uige?

He added that the spread of the disease would be "very dangerous and fast" because of cross-border movement of people

The implications of what is now being publicly said by this WHO representative does not look good. They are not talking like they think they have it contained.

FIFA postpone World Cup qualifier over virus fears

Fri May 13, 2005 12:09 PM GMT+05:30
LUANDA (Reuters) - A World Cup qualifier in Angola has been postponed because of fears over an outbreak of the Marburg virus which has killed nearly 300 people in the north of the country.

The African zone Group Four match between Angola and Algeria, scheduled for the capital Luanda on June 5, will now be played on August 17, FIFA have told the Angolan Football Federation.

But it could be moved to a neutral venue later in the year if the epidemic is not brought under control.

The Angolans are two points behind leaders Nigeria in the group, with Algeria in bottom place after failing to win in their last six matches.

The Angolan Football Federation on Friday criticised the FIFA decision, highlighting the distance from the epicentre of the outbreak to the capital. There have been 277 casualties, and more deaths are feared, in the northern province of Uige, the World Health Organisation (WHO) said last week.

The rare haemorrhagic fever is transmitted through bodily fluids including blood and saliva.

ESA helps during Angolan Marburg outbreak
May 14, 2005, 04:01 GMT

printer friendly email this article

PARIS (SPX) -- World Health Organisation personnel combating an Angolan outbreak of the lethal Marburg virus used high-resolution satellite-based urban maps provided through a pair of ESA-led activities.

The Marburg virus causes Ebola-like internal bleeding in humans, with an incubation period of between five and nine days. A Marburg outbreak was detected in Angola's Luanda city at the start of April.

It has since taken the lives of more than 255 victims, many of them children under five.

To help local World Health Organisation (WHO) teams fight the epidemic, updated maps of Angolan cities based on 2.5-metre resolution SPOT 5 imagery along with metric IKONOS images were prepared.

The maps have been used to orientate field workers, plan activities and integrate information on the spread of cases.

"This product will be of great help to our organisation with the recent outbreak of Marburg virus," said Johan Lemarchand of WHO. Latest reports indicate the virus is now under control.

The urban maps were provided through the ESA-led activities Epidemio and Respond. The first is a Data User Element project to supply satellite data and products to epidemiologists exploring links between the environment and disease.

Respond is a consortium dedicated to supplying satellite-based geo-information products direct to humanitarian users in the field, with partners including UNOSAT, an in-sector service provider within the United Nations community as well as the German Aerospace Centre (DLR).

Respond has been established as part of the pilot services of Global Monitoring for Environment and Security (GMES), a joint initiative between ESA and the European Union to build a global monitoring capability in support of Europe's environmental and sustainable development goals.

On behalf of Respond, UNOSAT in collaboration with the United Nations High Commission for Refugees (UNHCR) and with DLR provided WHO with a map of the city of Cabinda, UNOSAT also providing a map of Uige, capital of the province at the epicentre of the epidemic.

At the same time, Epidemio partner company C-CORE has supplied a map of the Angolan capital Luanda.

Both UNOSAT and C-CORE stayed in close contact with WHO, organising a makeshift partnership to ensure reliable and timely support for both activities.

OMS OPPOSES the FIFA ON MARBURG
2005-05-13

It defends the revision of this decision. The World-wide Organization of Saúde (OMS) stated its position this morning to the National Radio of Angola (RNA) for the voice of its representative, Fatoumata Diallo.

"We wait that the FIFA contacte the OMS and we review its position, if already he took it because he does not have risk to have problem of Marburg next to the athletes and all the delegation", the representative of the OMS in Angola declared.

Fatoumata clarified the position of the OMS on the decision of the International Federacy of the Associations of Futebol (FIFA) to change the date and the place of the game between Angola and Algeria due to the Marburg.

Luanda yesterday confirmed the decision that notifies it the FIFA in letter, when announcing to the press its ackward reaction.

"Even so Algeria has made this communication to the FIFA, to request to change the date and the place of the game, alleging the sanitary question of the virus of Marburg, us we waited that the FIFA had another reaction", it yesterday indicated the General Secretary of the Angolan Federacy of Futebol (FAF), Augustus Hisses Avarito.

Da Algeria recognizes the right "to use all its cunnings, also the dirty game", but exception: "risk country costuma to be Algeria, with the fundamentalists, the terrorists whom they assassinate and they slaughter for there, zone of seismic risk, terramotos".

For itself, "the FIFA is not nothing before a Nation", although to be "a powerful multinational, (that) it generates much money, puts into motion millions, has great traffic of influence".

The Angolan government must, therefore, in the point of view of this porting commentator, to talk back asperamente to the FIFA, being said to it "Or the game if plays in 5 day or not valley the penalty".
