I've waited several minutes over a high speed connection, and no document. Could you post the abstract?
It's a 27 page review. I'll try to get the introduction up but it'll take a while since "select all" in my version of Adobe Reader isn't doing that so I can copy and paste. I'll have to find a computer with Acrobat to get to the text.

I'll have to find a computer with Acrobat to get to the text. That won't help, unless Acrobat has a built-in OCR. The PDF is simply photographic images of the pages, not the actual text-and-formatting from which the pages were created.
Perhaps the author wasn't keen on being quoted...
