“rebar spacing”
There was a 15-story building in Seattle. Somebody forgot to coat the rebar ends (or perhaps it was coated cables) where they cut it. And I guess numerous inspectors forgot to check.
A few years in they noticed the rust. Millions of dollars of repairs couldn’t fix it. They had to tear down the 5(?) year old building.
