Yeah, standard distribution construction puts the neutral/ground wire below the phase conductors. This is partly because it also serves as the neutral conductor in single phase and 3-phase distribution wires. Your typical 120/240 service uses it as a neutral whereby you have 120 volts from one conductor to the ground, 120 from the other conductor to ground, and 240 from each of the 2 conductors. That’s in a single phase setup.
In sub-transmission, it is often above to act as a shield. Properly grounded, the shield wire attracts lightning (hopefully) and takes it safely to ground, leaving the primary conductors relatively safe. It provides a better path to ground and serve to shield the power lines below.
In high-voltage transmission systems, there are usually 1 or 2 ground wires that serve to provide a common bond all towers together and to ground, and again shield the main phase conductors of the actual transmission line. They also, in more recent times, have a fiber optic communications cable attached to one of the shield wires (OPGW, or Optical Ground Wire) to facilitate communications between substation terminals and other uses such as third party comms.

Yes, I read that. It is an unusual configuration and not used in my area. If it breaks and hits the 'Hot' lead then it is hot. The picture does not show a separate ground / neutral lead. In the picture the attachment to the pole looks fragile. The configuration is admittedly unknown to me. I have been a licensed electrician - but quite a while ago.
