You are right, that's not enough niacin for therapeutic effects. He needs to get up to 3g per day for one month. What he's done is basically a maintenance dose. Adding 1/2 tsp of cayenne or habanero (or hotter - the higher the Scoville rating the better) powder with each niacin dose will also help immensely. If he wants to stay on the stains, just add the pepper. I'm starting to think the capsaicin in peppers might be a magical blood thinner and particularly helpful in facilitating the distribution of nutrients.
