Much ado about nothing. Plenty of images on the ‘net showing his slightly cleft chin. The harsh front lighting and angle are just emphasizing his aging face, despite the plastic surgery he must be getting every so often. I don’t doubt that all presidents have a double for emergency purposes. Possibly the better way to tell one from the original is that the double might not seem so mentally vacant.

The ‘ball chin’ is maybe a third of a size of his actual chin - I can see skin/fat going away or drooping but not bone.
That’s not the same nose. Maybe he had work done I don’t know.
