What exactly is an "unwitnessed incident?" I assume it simply means "we think he fell in?"

I assume it is similar to unwitnessed death, which has legal ramifications. A person who is on hospice and has an unwitnessed death will not be autopsied but an unwitnessed death by someone not on hospice may be subject to autopsy.
