Investigators found that the facility was lacking legally required guardrails and restraint systems to protect workers from falling into dangerous equipment. Its difficult to believe the explanation is this simple. I certainly don't trust any media, particularly the low IQ clowns who work in local TV news, to give an accurate reporting of events. I wonder what facts they are not reporting?

This facility is probably built in the 50’s and to an earlier standard. Usually, unless an accident on site happens, they won’t see any health and safety inspectors.
I’m guessing that is what happened. 60 years of no incidents until now. Now the government is inspecting the facility.
It will cost millions to bring the facility up until code.

If the facility manager is not up on OSHA, yeah, I could see that happening as described.
When I started at one property, I insisted on installing a fall prevention device on a shipman’s ladder.
Why? So no one fell to their death.
