Some believe that the speech, in addition to being bizarre and insane, was crafted by his handlers to be veiled in such a way that, for example, Biden poses a problem(e.g., Charlottesville) and Biden then proposes to 'cure' that problem (e.g., 'end cancer'). So MAGA is the cancer that Biden is proposing to cure. But it's veiled under a medical reference that seems appropos of nothing.
Without all the veiling, it seems the core of Biden's Nuremberg rally is that he is warning people not to stand with those who expose election cheating (threatening to deter them with 'actions'), but is instead asking people to take a stand with him in ending the 'cancer' threatening our very souls (personally, I think that means planned riots, but I'm cynical). As more people align with MAGA values, the cancer spreads and Biden vows to 'end cancer as we know it'.
Overall, Biden's big picture communications are something like, "If you don't support me, run from me because I have F15s. If you do support me, take action to stop those questioning election fraud or else. MAGA is the lethal threat and I am the cure."
