The girl on set who was supposed to be in charge of the firearms is clearly going to take most of the blame, and should.

IMO she should get a third of the blame. There are three areas of negligence - the armorer, the actor who violated just about every gun safety rule, and the producer who failed to maintain a safe environment on set.

Two of those are the same person.
