If it wasn’t used before, then that speaks volumes.

This is typical death reporting. Don't get the idea that there is anything unusual about this terminology just because some non-scientist, non-medical blogger tells you it's unusual.

Essentially, always. The determinations rely on the expertise of doctors to recognize a set of symptoms, and only sometimes involved disease tests.
