You might tell him that for a man, taking the lower position (facing up) with you on top (ride 'em cowgirl!) puts far less strain on his heart. The usual position is with you facing him, but it can be done with you facing away (so he sees your back) as well. You're doing the "work" so he just lies there and enjoys. Risk of heart attack is negligible.

I’ll tell him but sometimes I feel like the problem goes deeper than that.

I make him laugh and he says he enjoys time with me, but that’s the extent of it.
