Many years ago I ran across one that I can't find right offhand, but it was like this:

Title bar: "Unrecoverable System Error"
Icon: Steaming pile of poo, complete with buzzing flies
Message: "You really don't want to know what just happened."
Three buttons: "Abort", "Ignore", and "Trash My FAT".
