At least in Iowa, wide scale testing and people live spread out. The typical Iowa small town is ~2000 people with a boarded up main street (few small businesses, few restaurants, etc). Buying mail order or from online is the norm. Making a monthly trip to the "city" to stock up at Walmart or Target is the norm. The most frequent local complaint is people can't go to church in a building. But churches are experimenting with online or parking lot services.
