It's becoming a problem in New York City because of the high residential density of Manhattan, Bronx, Brooklyn and Queens, which makes spreading the disease very easy. Even New York Governor Andrew Cuomo had a "Jesus moment" and now aggressively pursuing testing and medical help in the New York City boroughs.

Id sure like to know how many street people are getting hospitalized in NYC, LA, and some of the other hard hit cities.
this would be one heck of a time to turn the local gangs that run neighborhoods around. Delivering food, fixing yard fences, spraying streets, cleaning elevators and hallways of apartment buildings, giving them personal value.
All western democracies have large elderly populations. Including Japan, yet they fared much better than Italy has. It’s not as simple as “sick old people disease”.
