That (flushing) would be my second guess, I thought a small vacuuming tube would be the first tool to be used in this case.

Actually, Lidocaine. Cheap, effective, readily available in every ER.

Not necessarily...it is probably more due to what the physician has close to hand quickly. Apparatus for vacuuming ears is somewhat specialized to ENT sorts (for wax removal), but any physician has access to a syringe and cup of water.
