"The aircraft had been in maintenance and the maintenance log noted that the left hand thrust reverser was inoperative," Bruce Landsberg, vice-chairman of the US National Transportation Safety Board, told a press conference on Sunday.
Mr Landsberg added that shortly before they landed, the pilots had asked to change to a runway which had equipment set up on it, and which therefore had less space available.
"We don't know what they were thinking or why that was their choice," he said. Miami Air International is contracted by the US military for its twice-weekly "rotator" service between the US mainland and Guantanamo Bay, Bill Dougherty, a base spokesman said.
Officials say the people on Friday's flight included civilian and military personnel.

They flew with a dead thrust reverser?
A. I’m glad no one lost their lives during that difficult landing.
B. “Thrust Reverser” sounds like some type of retro-active birth control.
And just so everyone knows there were three fatalities, just not human ones...Pets died on the flight.
Landing distance calculations aren’t predicated upon working thrust reversers so that shouldn’t really be a factor. It’s quite common for thrust reversers to be inoperative.
“And just so everyone knows there were three fatalities, just not human ones...Pets died on the flight.”
In other words: THERE WERE NO FATALITIES.
Ditto.
God’s grace is amazing.
“Its quite common for thrust reversers to be inoperative.”
Really? I very much doubt that! And given that they were landing in inclement weather with a lot of water on the runway, the thrust reversers would have been an important “feature” in getting the plane stopped.
Onto a construction shortened non-grooved wet runway known for ponding, in heavy rain??
A lot of people view pets as other people and are willing to risk their lives to save them, just as if they were saving a child. Just last week a Brazilian model drowned trying to save puppies that fell into the water. These same people get a terrible dose of reality when something like this happens to their pets and they find out they are entitled to little or no compensation for their loss.
There used to be a TV show called The People's Court and that was a pretty common case, people suing for damages to their pets only to walk out in a state of shock when Judge Wapner told them their animals had no monetary value.
It's a cold, cruel world.
Onto a construction shortened non-grooved wet runway known for ponding, in heavy rain??
I should have added: With a gusting tail wind...
The report on this one does not sound like it will be kind to the Captain.
It isn't like they tried to do a landing and the thrust reversers failed on them. They knew they were inoperative, they knew the weather conditions and between the pilots and the air traffic controllers they knew what the runway was like. All of those factors should have been taken into account before the landing started, so they are probably looking for another cause.
If all this is true, that Captain might be out of a job. In a crash, there usually isnt just one cause. Its almost always a comedy of errors. It looks like that was true here too.
What if the dogs were LEO guard dogs?
ok.
Try landing in Charleston West Virginia with no reversers. and you will roll off the mountain.
A guy from the old neighborhood and his Co pilot died last month at Sundance airport when the left side thrust reverse deployed late in the final approach. Jet suddenly climbed then flipped over. Since then I’ve been wondering if this is simply a matter of grappling the wrong lever or was some interlock non funcitional?
The first part of your comment is correct but its not common for thrust reversers to be inoperable. They want you off the active as quickly as possible.
In that case I'm pretty certain thrust reversers were required. Oh and airline service at Coleman A. Young International Airport ended in 2000.
I thought it a bit strange that this police officer was dressed up as a pilot, and then I realized he must have been a plane clothed officer.
