It's probably related to obsessive-compulsive disorder. Temptations become almost almost impossible to resist.

Assuming they might want help, I doubt there is a known effective treatment for it.

Underwear fetishes are not real uncommon; at least I heard of it before.

Everyone has quirks and preferences. It's a matter of degree and just how much of a hold you a person it gets.
