There are two paths - the DoJ will formally receive the report from IG and then forward to Congress. They (Sessions + Wray) may include something explaining what they have done to address any issues or recommendations in the IG report; note there are actually three IG’s in play - Horowitz, NSA (which Rogers initiated and then referred to FISC acknowledging there had been illegal intercept requests) and one other, I believe under DNI but don’t recall. The IG report will go to congress with over 1.2 million pages of supporting documents & a public referral + findings will be made on the oig website. If DoJ does not act, then statute provides that Congress can then return referrals to them.
Sessions is recused on most of these issues. At this point in time it's still Rosenstein.

Not sure what you mean. Congress can return criminal referrals to the DOJ regardless of what the IG does and what the DOJ does.
