With all of the boat accidents that seem to be happening within the last year. How is it that the Captain’s, or the people that are directing the craft are never named?
Something smells.......
It took me a bit of time to read the entire report, but it was very informative. There was clearly a significant amount of detailed information recorded from the logs, recordings, and instrument readings that they were able to use to create a very detailed time log of the events leading to the incident. I was amazed at how many human errors were made in a short period of time, and there was a seeming lack of the other senior officers to advise the CO that he was making wrong decisions. It will be interesting to see if the detailed reports will be released once the investigations are done on the Fitzgerald and McCain collisions, or if that information will be classified.
