Desperate for hormonal treatments, transgender patients were turning to friends and illicit dealers for unregulated cocktails of drugs. Many were left with health complications and infections that landed them in the waiting room of a busy South Los Angeles clinic.
"For the most part, they were basically getting care on the streets," said Jim Mangia, president and chief executive of St. John's Well Child and Family Center, which runs the West 58th Street health center as part of a network of low-cost Los Angeles-area clinics.
After years of dealing with medical problems created by what he saw as a major gap in the healthcare system, Mangia created a dedicated transgender medical program at the nonprofit South L.A. facility that now serves more than 500 patients. Such specialized care "just wasn't there before" for these patients, he said.
Indeed, Alexa Vasquez, who traveled four hours by bus for an appointment at St. John's on a recent weekday, said that when she began transitioning two years ago, she struggled to find an affordable clinic offering the care she needs. Friends offered her what they said were hormones, but she was scared to take them.

Hint to mentally chall...er...transgendered “people”: If you go to the doctor’s office dressed up as a person possessing a gender that you are NOT, don’t expect the highly educated physician to personally regard you as anything but a candidate for the loony bin. He or she will most probably treat you as needed...regardless of your silly little micro agression imaginations.

One can change gender, although one can’t always get others to go along with it. One cannot change sex, no matter what.
That’s not the issue. They aren’t claiming that they’re having trouble getting flu shots, antibiotics, or treatment for a broken leg, regardless of how silly they dress or act. The “health care” that’s “difficult” is measures to facilitate their impersonation of the opposite sex.
Of course it’s difficult.
The doctor doesn’t know whether to prescribe a Pap Smear or a PSA test.

That is not entirely true, depending on how you try to define the meaning of sex. For example:
Hormonal sex. Yes, natural and artificial changes in the balance of sex hormones changes secondary sex characteristics and hormonal sex.
Chromosomal sex. Yes and no, some natural somatic chromosomes result in changes of morphological sex, mental sex, and various secondary sex characteristics during prenatal and/or post-natal development. Mosaic sex chromosomes produce a person who is both female and male at the cellular level, where some cells carry female sex chromosomes, some cells carry male sex chromosomes, and the morphological sex characteristics may be predominantly female, male, or a blend of female and male. Such mosaic chromosomal sex may not be subject to chromosomal sex change, yet may be subject to hormonal or other types of sex change.
Morphological sex. Yes, natural and artificial processes can change morphological sex, mental sex, and various secondary sex characteristics.
There are additional categories of sex definitions not used in the above examples.
