Flak jackets are designed for just that...flak, shrapnel and grenade fragments. They are not designed, nor intended to stop direct fire, small arms projectiles...

If it was an old Vietnam era flak jacket,as I suspect, it is not a bulletproof vest to begin with. That plus degradation of kevlar due to age.

He now has a hole where his heart was.

Maybe the vest was loose, or worn low, or she shot from a bit to the side.

Maybe he wadded it up over a less critical area and neglected the instruction to shoot into the vest....
