A shoe bomb 'dud' might start a fire and this could explain why the pilot climbed to 45,000 ft, to try to extinguish the fire. If the aircraft filled with toxic smoke or fumes, perhaps everyone went unconscious and the craft turned into a 'ghost flight' to nowhere.
