I would expect that what the Dr is talking about is giving enough medication to keep the child comfortable which is often a small degree of tolerance with the amount that is lethal.
Parents will often want their dying child kept comfortable and pain free, at a risk that the child might die a day or two early.
Personally I do not want my children to suffer from the effects of a terminal dying. I would rather they be kept pain free, even if it compromises their respirations in the long run.
