OK, so to those of you with DVRs etc:
In that final desperate, scrum at the end of extra time, prior to the Suarez handball, wasn't a Ghana player (#10 specifically) offside on a when the ball was headed toward him, or do I have the rule mixed up in my head? If so, it means Ghana got away with one before that handball, so it shouldn't have made it that far anyway.

I've seen it again on my DVR in slow motion (several times). And, I generally agree with your analysis, but I think #10 (Steven Appiah) *could* have been whistled offside when the ball was flicked on because he was near the goalkeeper and could be considered a player that has to be accounted for by the defense. There has certainly been less called before as offside. In this case, though, I do think it was a good no call as he didn't get involved until he was played back onside. Watching that play in slow motion is even more incredible than real time. Simply unbelievable how that ball didn't go in...Appiah had a wide open goal on his kick and happened to find the knee of the Uruguayan defender. What a way to lose after nearly winning in the dying seconds.

FIFA may ban Suarez for the Finals should Uruguay win against Holland. I think it would be just, since essentially he turned out to be a hero for that handball. I think because it happened so close to the end of the game, where Uruguay would not be punished by having to play a man down, that the punishment should be a bit more harsh than normally warranted.
You are talking about the difference between active and passive offside.
As has been mentioned elsewhere on thread, if a player is not in an ‘active’ attitude when the ball is played, meaning the ball is not being played toward him, offside is generally not given.

It is hard to determine offside from that picture since the ball is in the air, having already been played. Appiah could have run onto the ball, quite legally, in those circumstances.
From my crude screen grabs it really does not look like an offside on the first head on because the player in the middle was onside and the lower player (#10) was so close it was an OK no call, IMHO.
The photo below is a grab of what seems to me to be the earliest point of the first head on off a Ghana free kick.
The bigger question for me is if the player (#10) hitting the shot that bounced off the defender's leg was offside at the time. I can't tell from the video if the goalie punched the ball to him or if it was headed over by his Ghana teammate.
