This format in no way matches my son's Certificate, doesn't have a seal and doesn't have 'State of Hawaii, Department of Health, Research & Statistics Office' in the upper left hand corner.
Rather good facsimile but suspect.

The mother's and the father's signatures are not on the document, only Madelyn's as informant. that is actually the way a home birth would be filed. Although the mother could sign as the informant, and grandma as the witness.
I wonder who signed as the witness for my granddaughters' youngest cousin? He was born in the ambulance on the side of the road. The tech had of course never delivered a baby before. Fortunately it was Aunt C's third child, so she knew what to do. :)
