Most consumer driven plans have a deductable that must apply to hospital, doctors office, and drugs combined. Once met, many will pick up 100% of cost (in your PPO network). Deductables range from as low as 1800 to as high as 5000.
In California, I find that the $3500 is my most popular plan.

I would think at a minimum health insurance should cover one yearly physical, immunizations, and catastrophic services / insanely expensive medication..
