Cleaning up seriously infected Windows PC's is not easy. No one program is likely to do it. The last time I managed to clean up a seriously infected computer took me a days work, and a half dozen programs, most of which I paid license fees to use. It's usually easier to backup the files you care about and reinstall; though that isn't that easy either, unless you already gotten good with backups. And of course, it takes a carefully selective restore of your files, after the reinstall, to avoid reinfecting your PC from the backup.
One more program I'd recommend, that I didn't notice above, is Panda AntiRootkit Official Release.
The more thorough tests I've seen of anti-spyware software over the last year have all agreed that there is no program that can get more than perhaps 60% to 70% of what's out there. It's rather like medicines, where no one drug cures all diseases. This is a rapidly changing business, both the good guys and the bad guys. What was king of the hill six months ago is often an also-ran now, even though it has been constantly improved over the six months.

A story going around a year ago was that a neighbor of Steve Ballmer (of Microsoft fame) asked Steve to help clean up an infected PC. Steve gave up after a few hours, and turned it over to one of Microsoft's best engineers. The engineer gave up a few hours later, recommending a fresh new install.

The story is indeed true, except you're misremembering some details. Ballmer gave it over to a team of engineers who pored over it for a while and admitted defeat. They even came out and said they had no idea it was that bad. It tells you what kind of bubble Microsoft lives in.
