The painting is filled with symbolism to the point that there are whole books written. It's the first painting where the person places his name prominently in the picture.

The Black Plague would explain the pregnancy to a certain extent. During times of famine or disease, it is common for the sex drive to be about the only "human" feeling left...as a promise of continuation of the species.
