Here's an opportunity for an entrepreneur. An on-line subsidiary to a website with the word "Free" in it's title could offer a "Free" onlne permission slip. When this slip is executed it would be a legal document, (like a living will) and provide for this type of service!!!
It's been done.... sometimes free; sometimes not free; sometimes on stage. :)
