Found in BIOS Chipset Features Setup:
USB Controller: Enabled
USB Legacy Support: Disabled
USB Port 60/40 Emulation: Disabled
Changed USB Controller to Disabled, saved and exited. No effect; fault condition persists. Changed back.

I snuck in plenty of cigarettes in high school, but back then the teachers weren't too concerned with it. I'm old enough that we had a "smoking pit" at our high school. But even then the bathrooms were still always smoke-filled.
For four long years, you had the choice of occasionally smelling like smoke, or chosing not to pee between the hours of 8:00 a.m. and 4:00 p.m.

Try setting "USB Controller" to Disabled, and "USB Legacy Support" to Enabled, and then try the card again.
If that doesn't work, it may require a jumper setting too. Incidentally, do you know what version of USB is on the card?

Who gives a crap if the girl smoked or didn't smoke or smells like smoke or farts smoke signals?
If the worst thing she's done as a teenager is smoke a cigarette, then please Lord let me be so lucky with my kids!!!

I'll try enabling "USB Legacy Support" with "USB Controller" Enabled, just for kicks.

"Disabled",
"Keyboard/FDD",
and "KB/Mouse/FDD".
