A hospital with no MD in the ER certainly has no team on-call and nearby to repair an aortic transection.
Sorry about your brother but unless the fork lift hit him in the parking lot of the Texas Heart Center or NYU Downtown, or some similar place, that injury was not survivable.
My mother worked in hospitals in the late 60’s early 70’s and even then they had crazy stories...............

Is this always true, though? Some rural areas, you may wait 20-30 minutes for an ambulance, and it can’t drive as fast as a car to the ER, which might be 30 minutes away by speeding car, but 40 or 50 minutes away by slower ambulance.
