Transexuals come to realize that changing their observable sex traits does not make them happy. The “fantasy” never entirely becomes “reality”.
Some transition back.
Some commit suicide.
I read an account on-line, I can't find it now, but it was someone who fully transitioned from male to female and the excruciating pain and agony he was now dealing with that nobody ever told him about. Unable to walk, unable to sit, stitches, can't urinate, infections, and on and on. He was deeply depressed and regretted getting his dick chopped off almost immediately. When you go this far, there's no transitioning back.
The Hippocratic Oath states "first do no harm" and any doctor who would perform such a surgery needs to be stripped of their ability to practice medicine.

Transitioning only helps 50% of the patients. Acceptance as they are and lots of therapy should help 100%. No need to be chopping. No need to fake a gender change.
The Reality of the "Transgendered"
Lopping off your boobs, hacking off part of your ass, rolling it up in a tube and attaching it to your groin does not make you a "transgender male".
Lopping off your penis, having silicone balloons put in your chest and growing your hair long does not make you a "transgender female".
What it makes you is a mutilated, mentally ill individual who was surrounded by professionals, family and friends who, instead of helping you, pandered to your illness.
