There is no file number on the form and it does not look like it was whited out. The source of the document does not appear to give any background as to where it came from so I would guess it is another DU game document.

The signatures at the bottom appear to be all the same hand, just trying to make it look like different handwriting.
My mom was about the same era as Bamies gran’ma and signatures-especially by females- looked very different from that. Back then, penmanship was taught.
It looks kosher enough, but the siggies got me saying Nope.
Just my 1.3 cents.
