My guess is that he had varicosities in his stomach from a bad liver, and one burst.
That’s how my SIL went out. Like a horror show scene.
If the man was alcoholic, it may have been esophageal varices.

Due to damage to the liver caused by scarring from cirrhosis, blood is unable to flow properly. This can lead to a situation called portal hypertension. This increased pressure causes the veins in the esophagus to become distended and it can eventually burst.
The result is somewhat like projectile vomiting.

my worse situation while I was working as an RN in an acut care hospital was this guy who had terminal cancer and was going to go home in a day or two, cancer of the lungs/esophagus IIRC..
he was sitting up for breakfast, had his usual cough, then got into bed and started coughing/vomiting up massive amounts of blood and it went everywhere..all over the bed and room....a code was called but it was decided that he and his family wanted a no code and the man thankfully ended his life without having to be intubated,sedated, and wrist restrained....it was the worse scene I have ever seen....

Yep. Blood accumulates in your stomach, then you start throwing it up. Often violently. Aspirate badly and then it’s worse / you may well die.
Happened to both my Mom and Dad (although they both survived horrific episodes, with Dad succumbing to another months later as he was inoperable.)
