Massachusetts had a state crime lab employee mishandle (for decades) drug evidence for her own needs. (Substituting lawn weeds or baking powder as needed).

A Miami-Dade County forensic ballistics employee (now dead) simply reported that “the evidence was insufficient for examination.”

He used that “saved” time to write articles for his other (and somewhat-related) job.
