Pregnant women were never formally included in the initial clinical trials for Emergency Use Authorization (EUA) of COVID-19 vaccines in December 2020, meaning there was no specific "approval" for this group at launch. Instead, major health agencies shifted from exclusion to recommendation based on observational data and ethical considerations:
Initial Exclusion: Pregnant women were excluded from the original Pfizer and Moderna trials, leading the World Health Organization (WHO) and the UK's Medicines and Healthcare products Regulatory Agency (MHRA) to initially advise against vaccination for pregnant women in January 2021 due to a lack of safety data.
Policy Reversal: The WHO reversed its stance on the Moderna vaccine on January 29, 2021, stating that pregnant women at high risk could choose to be vaccinated.
US Recommendation: The CDC and the American College of Obstetricians and Gynecologists (ACOG) moved to "enthusiastically" recommend vaccination for all pregnant people by April and July 2021, respectively, despite the ongoing Pfizer pregnancy trial being halted early in July 2021 without publishing final results.
Recent Policy Changes (2025–2026): In a significant reversal, Health Secretary Robert F. Kennedy Jr. announced on May 27, 2025, that the CDC would stop recommending COVID-19 vaccines for healthy pregnant women and children, removing them from the federal immunization schedule pending new clinical trials. While the CDC advisers quietly expanded access again via "shared clinical decision-making" in September 2025, the official recommendation for universal vaccination of healthy pregnant women was effectively withdrawn in 2025.
