In the 70s and 80s getting 86'd meant getting banned from a bar. At least on the west coast that's what it meant. I never heard the Mafia association until recently.
It’s been restaurant lingo for years. I know this through personal experience. But that doesn’t mean it isn’t also a mob term for “eliminating” someone.
BTW, the term’s use in restaurants goes back almost a century, and may very well pre-date its use in mob lingo. I don’t know for certain if it pre-dates its mob use, but regardless, it’s restaurants for quite a long time, and anyone who’s worked in a restaurant is familiar with its usage.
