It could have been the captain or the first officer flying the approach. The other monitors and should take control in a dangerous situation. Both are in deep crap with the FAA. A foot lower would have killed the van driver and possible disabled the landing gear with then subsequent tragedy.

If the landing gear had hit the truck, the flight would have ended right there..............

This was a 9 1/2 hour flight. I am guessing there was at least a 3rd pilot in the cockpit and would have been in the jump seat for landing..
