“Bless their hearts” - Isn’t that a way for a woman in the South to say “Eat sh*t and die (painfully)” without actually swearing in polite company?

In this case, yes but not in all cases of that phrase. It can also be a term or endearment/love/sympathy/empathy/pity depending on the context and the person saying it.

I had an aunt who said, “Bless his/her little heart,” all the time and she never meant it as negative/derogatory.

Yes...and No. It all depends on context, tone of voice and timing. It’s often said in sincerity or real sympathy.

In the instance of Dr. “Chris Tina” it was clearly meant as a vicious insult.
