I would figure that Rail Stations could be engineered to prevent such incidents
by installing barriers that would only open when the train has stopped
in position to allow egress and access to the cars.

We need to reinstitute insane asylums for these folks. No more living on the streets Either they get involuntarily committed if they are a danger to others or sent to jail if they are druggies with a dozen different rehab assignments or they find a place to live. For the 2% that are normal but have fallen on hard times, gubmint should provide some assistance to get them off the streets
