i dont think so - but that FUBAR with that NOAA N-Prime satellite flopping over in 2004 made me think of that.

They didnt attach the lug nuts on the securing bolts and NO ONE did a visual inspection before tipping it over for attachment to the rocket and Plop! Millions squandered.

Apparently a clamp that secures a strap came loose and shook the observatory....................
