When the 'transgender; thing first broke into the national consciousness, in my innocence, I thought it was about those unfortunates who were "assigned" a sex by doctors shortly after birth. If only that were the case.

Since I was wrong about that I'm probably also wrong about this condition explaining Big Mike's mystery bulge. I've debated posting on it for a long time and resisted for the reason you cited in your post. What the eyes have seen cannot be unseen.
