My best guess is that she has had some serious body and fender work to the face, perhaps with implants and lots of extra strength, Pelosi grade botox.
Additionally, she probably has some of the collagen building particle filler injected. The filler is in a water base so when the filler is injected it simulates what you will look like 3-6 months down the road when the particles stimulate the build of new collogen.
In the short term, the water is resorbed leaving the particles behind, which produces a deflated look in the short term
