Fake murder & removal by operative
Real murder & later removal by regulars

could anyone, under cover of the 3 items listed above, have slipped him something for a -voluntary- demise?

Agree with all your points ... but I’m having more and more difficulty thinking the prison authorities would participate in a murder.
If they did, they’ll get caught, and they know it. So multiple people would have to be strongly under the power of the cabal.
That’s why I’m now leaning that the obvious “conspiracy” had to be for a LEGAL purpose—i.e., getting the prisoner into another, secret, location (for whatever reason) pursuant to a lawful order.
