Vuong, McPherson-Anderson and Ajarie failed to find a weapon kept by Joan Vasquez, an inmate. On September 2, 2016, Vasquez used the weapon to stab Plaintiff in the head. Ajarie was present for the attack but did not help Plaintiff or intervene. Plaintiff’s wound caused heavy bleeding, and Plaintiff passed out as he tried to seek medical attention.”
“When Plaintiff awoke, he was being taken to Brooklyn First Hospital. At the hospital, Plaintiff did not receive an MRI or X-ray.”

OK, what is a BP-8 prisoner complaint form? Did Epstein file a complaint form after being injured in july?? If so, where is it? If not, why not?

a BP-8 is apparently an informal request for remedy of a problem which must be submitted to the prison by an inmate within 20 days of an incident.
a BP-9 is actually a BP-229, a (formal) “request for administrative remedy.”
The BP-9 form cannot be filed unless preceded by the filing of a BP-8 form by the inmate against the prison and/or prison officials.
If Epstein had a serious beef and was not intimidated by the prison personnel, he or his defense lawyer would therefore most likely have filed a BP-8 immediately after the July incident, followed by a BP-9. I am not a lawyer, but according to my informal understanding of administrative law, all administrative remedies must be pursued prior to filing a formal complaint against the prison in open court.
