I remember some of the instructions said to answer the question whether you are a citizen or not.

I also remember the instructions for each box said something to the effect: In the name field, print name.

So I entered N A M E in the name fields.

I had some fun at the Docs office when they were asking some personal stuff, so I wrote:
Eyes-Two
Sex-Occasionally and so on!

I laughed at a lot of the forms. The earlier forms would have Santa letters, pictures of summer houses people were trying to rent, childrens drawings,bills, and you name it.
