In Ohio there are 14 required diseases that children have to have to attend school. Many of these are administered in combination vaccines. DTAP for example covers 3 diseases.

The 14 diseases are:

(1) Chicken pox;
(2) Diphtheria;
(3) Haemophilus influenzae type b; (4) Hepatitis A;
(5) Hepatitis B;
(6) Influenza;
(7) Measles;
(8) Mumps;
(9) Pertussis;
(10) Pneumococcal disease;
(11) Poliomyelitis;
(12) Rotavirus; (13) Rubella; (14) Tetanus.

However, the vaccines aren’t mandatory for the following reasons:

(1) A child is not required to be immunized against a disease specified in division (B) of this section if any of the following is the case:

(a) Immunization against the disease is medically contraindicated for the child;

(b) The child’s parent or guardian has declined to have the child immunized against the disease for reasons of conscience, including religious convictions;

(c) Immunization against the disease is not medically appropriate for the child’s age.

(2) In the case of influenza, a child is not required to be immunized against the disease if the seasonal vaccine is not available.

(1)(b) above leaves a lot of decision authority with the parent to decline any immunization on the basis of conscience - this could be religious or it could be because they feel the immunization is insufficiently tested. The school cannot mandate it by law.

Immunization required for 7th grade:

Tetanus, diphtheria, and pertussis booster (Tdap)
Measles, Mumps, and Rubella (MMR)

Immunizations required to enter Kindergarten:

Measles, Mumps, and Rubella (MMR)
Hepatitis B
Varicella (Chickenpox)

Immunizations required to enter Child Care (depends on age when enrolling):

Polio
Diphtheria, Tetanus, and Pertussis (DTaP)
Haemophilus influenzae type b
Measles, Mumps, and Rubella (MMR)
Hepatitis B
Varicella (Chickenpox)

NOTE: as of 2016 California changed their laws as follows -
Personal belief exemptions will no longer be permitted beginning January 1, 2016. Personal belief exemptions submitted before January 1, 2016 are valid until the child enters kindergarten (including transitional kindergarten) or 7th grade. Valid medical exemptions from a licensed physician are not affected and will continue to be accepted.
