I honestly wouldn't put something like that past the Democrats...
If October rolls around and Hillary is behind by more than five points. I wouldn't put it past them to try and pull a switcheroo and replace her with Biden or Warren. The thinking would be that they really have nothing to lose at that point and maybe some "fresh air" might change the dynamics of the race. The big question is "Would Hillary go quietly?" I suppose that would depend on whether the Clinton family decided it would be better for the dynasty to have Hillary remembered as the candidate who had drop out for "health reasons" rather than the candidate who got beat by Trump.
I don't think she would. We just saw that Cruz would not, and he had a lot less at stake than she does. The scenario of dumping her due to "health reasons" probably does appeal to some dem operatives; but I think she would hold on with bloody fingernails. Some people simply want it too much.

Hillary has felt it’s her destiny, her entire adult life, that she would be president someday. I don’t see her going quietly for “health” reasons.
Though it seems evident she has some health problems, she will never admit to same, and she would rather die trying to become president than bow out.
